CatherineGives.org is a nonprofit organization, partnered with and funded by Penn Medicine. Our mission is to connect healthcare organizations treating complex diseases in marginalized populations. Our vision of creating a city-wide hand-off aims to improve continuity of care, achieve better clinical outcomes for patients, and reduce administrative burden for providers.
Our Story
What happens when you combine a passion for improving care, relentless focus on usability and a team that truly believes that better technology means a better system for all? You get CatherineGives.org. Created at Penn Medicine to improve the EHR experience for clinicians by giving them a tool truly built for their workflow, CareAlign – originally called Carelign – quickly became the cornerstone of clinical care. Knowing the platform would improve care at other health systems, Dr. Subha Airan-Javia created CatherineGives.org, a non-profit focused on helping clinicians who care for underserved populations.
What we do
Sustainable change requires people, process and technology. Many dedicated and passionate individuals are working hard to provide better care to those in need. Two significant challenges they often face are: working in silos and inadequate technology to support their efforts.
Building on 15 years of experience designing, developing, and implementing clinical team collaboration technology at Penn Medicine, CatherineGives.org steps in to bridge this gap. We bring a combination of technology developed at Penn Medicine (CareAlign) and research based process improvements to community-based organizations, clinics, and other health care workers caring for individuals affected by poor social drivers of health (SDOH).
How we do it
We accomplish our mission by bringing three major pillars of sustainable change: process improvements, well designed technology and dynamic care planning to community based organizations. CareAlign is a digital teamwork platform that helps teams stay coordinated about patient care, addressing two major challenges.
Well-Designed Technology: Our platform, CareAlign, is user-centered and intuitively designed to simplify rather than complicate workflows. It reduces the administrative burden and allows healthcare professionals to focus on patient care.
Process Improvements: Well designed technology can’t, by itself, improve workflows for providers. We also need to create more supportive and collaborative processes to ensure everyone can focus on what matters most – the patient. CatherineGives.org’s mission is to incorporate well designed technology and process improvements to improve outcomes for underserved populations.
Dynamic Care Planning: In CareAlign, the platform provided by CatherineGives.org, each patient has their own “wiki” or shared document for collaborative care planning across institutions. Unlike a progress note or discharge summary, which are fixed at a certain point in time, this is a living, dynamic document accessible to all care providers, ensuring everyone involved in a patient’s care is on the same page. This reduces gaps and delays in care for patients who are frequently admitted to different hospitals and do not have consistent outpatient care.
Subha Airan-Javia, MD
Founder, CEO
As a hospitalist with Penn Medicine, Subha recognized the daily challenges clinicians faced due to inefficient workflows and poorly designed technology. Knowing there was a better way, Subha and her team created Carelign, a digital handoff solution. After Carelign revolutionized the handoff process at Penn, Subha knew the platform could bring the same value to other institutions – which is how CareAlign came to be. Subha now spends her days working with various clinicians and healthcare administrations to bring CareAlign to the wider market. As the Founder and CEO of CareAlign, Subha brings over a decade of research on the intersection of technology and clinical workflows, as well as experience as an Associate CMIO at Penn Medicine.

Meghan Cintorino
VP of Marketing
As a marketing professional with over 10 years of experience in SaaS Tech companies, Meghan joined the team with a mission to help get CareAlign into the hands of as many clinicians as possible. Familiar with the challenges faced by the healthcare industry from the patient perspective, she brings a healthcare consumer point of view and a background in tech marketing to the team. Meghan is a proud Temple University alum with a bachelors degree in International Marketing & Management Information Systems.
Tristan Dooley
Director of Operations
Tristan has experience in a variety of different positions across various healthcare organizations. She is interested in improving the health and wellness of both patients and clinicians. Tristan has a Bachelor’s degree in Psychology from the University of Wisconsin (Go Badgers!) and a Master’s in Healthcare Administration from UW-La Crosse.
Joel Kahn, MD
VP of Business Development & Chief Privacy Officer
As a physician involved in the business world, Joel has worked with multiple companies involved with different aspects of healthcare. From Insurers to international consulting, tech startups and digital therapeutics he has always tried to bring the latest tools and technology to consumer and professionals in a way that improves their work or enriches their life. At eMindful he helped developed mindfulness programs to help physician deal with the effects of burnout and CareAlign continues that trajectory by focusing on tools that prevent burnout and give time back to clinicians’ overtaxed life.
