Equitable care for all patientS
Despite the dedication of clinicians, our healthcare system still falls short in providing equitable care for all patients. Many individuals struggle to maintain their health due lack of transportation, community resources, insurance, and financial resources, making it difficult to access preventive care and manage chronic conditions. As a 501c3 non-profit, we believe that high quality healthcare is a basic right and we are working with community based organizations to improve care for marginalized populations.
Creating a city wide handoff
Marginalized patients often receive care in multiple locations, making it difficult to share information and important follow ups from previous encounters across providers. To ensure no one falls through the cracks, providers need a better way to get the full picture of a patient’s health – where have they received care? What tests have already been done and don’t need to be repeated? What key results are we waiting for? What treatment plan is best for this individual?
Creating a City Wide Handoff will require a better process for managing patients, dedicated people to care for underserved individuals and technology to support the effort.
